The chances of the world holding temperature rises to 2C – the level of global warming considered "safe" by scientists – appear to be fading fast with US scientists reporting the second-greatest annual rise in CO2 emissions in 2012.

The Pentagon sent a US veteran of the "dirty wars" in Central America to oversee sectarian police commando units in Iraq that set up secret detention and torture centres to get information from insurgents. When the local doctor who had been treating Vicky Hilborn told her that her rare cancer had spread throughout her body, including her brain, she and her husband refused to accept a death sentence. Within days, Keith Hilborn was on the phone with an "oncology information specialist" at Cancer Treatment Centers of America.
